Risk management is often associated with formal processes. Risk registers. Periodic reviews. Structured assessments carried out at set points in time. Those artefacts matter. But they are rarely where risk actually starts.

In more mature organisations, risk is understood as something that emerges continuously, through day to day work, decisions and trade-offs. The risk register is not the work of managing risk. It is a reflection of it.
