Complexity in organisations builds gradually, through decisions that are individually sensible. A team identifies a bottleneck and introduces a tool to address it. Work becomes faster, visibility improves, and something that used to take hours now takes minutes. The outcome is positive, so the pattern repeats.
What tends to emerge later is a different problem. The decisions themselves remain sound, but the way they accumulate starts to change how the organisation functions.
